As a courtesy, our staff will help you determine your insurance benefits. In addition, we will be happy to submit a claim for the services provided. Most insurance companies will respond in 4-weeks. Any balance remaining, after your insurance pays, is your responsibility. Your prompt remittance is appreciated.

You are fully responsible for all fees charged within 60 days of service, regardless of insurance reimbursement.
