As a courtesy OSA will file claims with any insurance plan even if we are not contracted with that plan and that plan doesn’t require the patient to see one of their participating providers.

OSA is contracted with the following insurance plans:

• Alpha Dental - 20% discount plan, patient must pay in full at time of service.
• American Dental Group – 20 or 25% discount plan, patient must pay in full at time of service.
• American National Dental Services – discount plan requires payment with service.
• Ameritas PPO
• Blue Cross Blue Shield of CO (not medical or all BCBS dental plans)
• Careington Care 15 Discount
• Cigna HMO - must have referral, patient has a co-payment
• Cigna PPO
• Delta Care- must have referral, patient has a co-payment.
• Delta Dental of CO (not DPO or EPO) - usually insurance pays 80%.
• Dental Classic - usually insurance pays 80%.
• DenteMax
• GEHA Connection Dental Network
• Guardian DentalGuard PPO
• Humana Dental PPO
• Mutually Preferred
• PacifiCare Dental PPO
• Provider Dental Plans
• Safe Health PPO
• Total Dental Admin. PPO
• United Concordia - usually insurance pays 60%

For these plans a patient should call insurance to verify they can be seen at OSA.

OSA is not contracted with: (OSA will still see these patients on a fee-for-service basis)

If a patient is being seen for TMJ, implants, or pathology, these services will probably fall under the patient’s medical insurance. The patient should call their insurance plan to verify if OSA’s services will be considered a “covered benefit.”
